These Terms & Conditions govern your use of our website and requests for mobile phlebotomy and specimen collection services. By using this website or requesting our services, you agree to these Terms & Conditions.   ## 1. Our Services   CertiScreen Health Lab LLC provides mobile phlebotomy and specimen collection services. We travel to approved locations to collect specimens when appropriate and coordinate the collection process according to the requested service. CertiScreen is not a physician or medical practice and does not diagnose medical conditions, prescribe medications, provide medical treatment, or independently interpret laboratory results. ## 2. Laboratory Orders   Certain blood collection services may require a valid laboratory order from an appropriately authorized healthcare provider or laboratory. You are responsible for providing any required laboratory order or arranging for the appropriate provider or laboratory to send the order through an approved method before your appointment when required. CertiScreen may decline or postpone a collection when required information or documentation has not been received.   ## 3. Appointments and Availability   Submitting an appointment request through our website does not guarantee an appointment. Requested dates and times are subject to availability. Your appointment is not confirmed until you receive confirmation from CertiScreen Health Lab LLC. We may contact you to verify appointment details, service location, laboratory order information, or other information needed to coordinate your collection.   ## 4. Mobile Service Location   You are responsible for providing an accurate and appropriate address for the requested mobile blood collection. The collection location must provide a reasonably safe, clean, and suitable environment for the service. CertiScreen may decline or discontinue a service when conditions create safety, access, or collection concerns.   ## 5. Fees and Payment   Applicable collection fees, mobile service fees, travel fees, or other charges will be communicated as appropriate. Unless otherwise stated, CertiScreen's fees are for the mobile phlebotomy or specimen collection services provided by CertiScreen. Laboratory testing fees, healthcare provider fees, and other third-party charges may be separate.   ## 6. Cancellations and Rescheduling   If you need to cancel or reschedule an appointment, please contact CertiScreen as soon as possible. Cancellation, rescheduling, travel, or other applicable fees may apply when disclosed to you in connection with the service.   ## 7. Laboratory Testing and Results   Specimens collected by CertiScreen may be delivered, transferred, or otherwise handled according to the applicable laboratory or service arrangement. Laboratory testing is performed by the applicable laboratory, not by CertiScreen unless expressly stated otherwise. Laboratory results, medical interpretation, diagnosis, treatment decisions, and medical advice should be addressed with the appropriate laboratory and/or qualified healthcare provider.   ## 8. Website Information   Information provided on the CertiScreen website is for general informational and service-coordination purposes. Website content is not intended to replace professional medical advice, diagnosis, or treatment.
