Request Your Blood Draw  Complete our online appointment request form and select the service you need, your preferred date and time, and the location where you would like your blood draw performed.  ###  2. We Confirm Your Appointment   A member of the CertiScreen Health Lab team will review your request, confirm availability, and contact you to finalize your appointment date and time. Your appointment is not confirmed until you receive confirmation from CertiScreen Health Lab.    ###  3. Prepare for Your Blood Draw   Have your provider-issued laboratory order available if required. Follow any preparation instructions provided by your healthcare provider or laboratory, such as fasting instructions. CertiScreen does not independently determine your testing or preparation requirements.    ### 4. We Come to You   A CertiScreen phlebotomist will arrive at your confirmed location to perform your blood specimen collection. Your specimens will be handled according to the requirements for your provider-ordered laboratory testing.  ###  5. Your Specimen Is Sent to the Laboratory   After collection, your specimen is prepared for transport to the appropriate laboratory for testing according to the laboratory order and applicable handling requirements.
